Frequently Asked Questions (FAQ)
- Who is eligible? Any adult over the age of 18 who has 1 or more medical, cognitive, or mental health condition OR any adults who require daily protective supervision by health professionals.
- How will I pay? Our program is FREE with Medi-Cal coverage (Health Net, Kaiser, Molina, Anthem Blue Cross, LA Care, Blue Promise, etc.)
- Will this program affect any current or future benefits? NO, our program does not affect any current or future benefits you may be receiving.
- How do I get started? 1) Schedule a visit or simply walk-in. Please bring your ID and Medi-Cal Insurance card. 2) Ask your Primary Care Physician (PCP) to fill out the medical information packet we provide for you. 3) Bring back the medical examination packet for review and the admitting process will begin.
- Do you provide transportation? Yes! We do provide transportation to and from the center. If you live farther away you may qualify for Access Transportation and we can assist you in applying.
- Do your clients live at the center? No, our participants do not live at the center. We are a day program and our participants are at the center 4 hours a day then they go back to their respective homes.
- What languages are spoken at the center? Our employees speak English, Spanish, and Tagalog. Our participants speak either Spanish, English, Tagalog, or Mandarin. We want everyone to feel included and we try our best to break any language barriers.
- What is your service area? We serve many cities which include: Baldwin Park, El Monte, Covina, La Puente, West Covina, Pomona, La Verne, Azusa, San Dimas, Walnut, and Glendora.
